I believe my interest in the field was shaped by my family’s experience dealing with the challenges that come with terminal illness and advancing age. I am humbled to have been part of their journey as they faced death with dignity and grace.Living in a small community presents significant challenges that may never occur in a large urban setting. The team you work with knows your family or the names of your children because in their spare time they are the piano teacher or soccer coach for your children, friends that knew you growing up. This close knit atmosphere can be a blessing when things go well and unbearable when mistakes are made or professionals disagree, because you still interact with one another outside the context of your professional role and this is when moral distress begins.I began working with A. J. ten years ago to facilitate the completion of a health care directive. At the time A. J was beginning to show the signs of a neuro–muscular disease experienced by other members of the family. In early 2000 most directives were fairly straightforward with the option to choose artificial nutrition and hydration or not. A. J. very specifically checked “none.” Years went by living independently for A. J. but as most illnesses progress so did A. J’s, and admission to a health care institution became necessary. In many situations medically assisted nutrition and hydration would be indicated, but in this case the wishes of the patient were known in the form of a health care directive. The decision makers inability to honor the directive was the purpose for my involvement almost a decade after my initial work with A. J.No matter where you stand in the debate regarding medically assisted nutrition and hydration being burdensome or ordinary care, most people believe in the importance of a health care directive given the histories of Nancy Cruzan, Karen Quinlan and Terri Schiavo. My objective was to compile the legal information that would elucidate the patient preferences and help staff and the decision maker understand it was time to honor the directive given the advanced stage of disease. Numerous attempts were made with the surrogate and health care professionals to help them understand state statutes on health care directives supporting a patient’s right to chose. It became clear that some of the team seemed disinterested in my concerns or my responsibility as a professional, and I began to believe the difficulty was a failure on my part to communicate clearly.During that first year conversations and meetings were scheduled with the patients decision makers and they felt unable to make a decision and requested the entire family provide input. Although they understood A. J’s wishes, they felt everyone should be in agreement. In my experience when leaving decisions of this magnitude to a large extended family, communications deteriorate and little can be accomplished. Some members in the family held fundamental religious beliefs and thought a miracle could change the prognosis. While I believe as health care professionals we must respect individual religious beliefs and values, we are also morally obligated to advocate on behalf of our patients a fundamental principle I hold dear.The health care team was no more eager to address the issue than the family. I had produced specific state statutes regarding the health care directive of a previously competent person and even in light of this information, some individuals continued to believe the surrogates decision took precedence over the directive. This growing tension left me feeling angry and ineffective, questioning my role in the organization, while watching A. J. linger in a state not chosen. I believed I had failed... (shrink)

This book presents and exemplifies the theory of grammar called Semantic Syntax. The grammar, which offers a syntactic theory closely connected with semantic analyses, is a direct continuation of Generative Semantics; it will re-ignite interest in that framework which flourished and promised so much in the 1960s and 1970s.

Plamondon & Alimi's model will gain substantially in credibility when it is able to come up with predictions for new (rather than old) experimental results that discriminate between various models. Moreover, the present model is nothing more than a descriptive not an explanation for motor performance. A link to the contribution of various neuronal mechanisms involved in motor control and of muscle properties to the performance of the model is crucial.

Suffering Belief reviews the leading attempts at justifying the existence of evil and salvaging a rational basis of belief in the traditional Western God. The author raises the ethical problem of the widespread use of drugs such as Prozac, among individuals with normal mental disorders, such as nostalgia or discomfort. Referring to the work of practitioner P. Kramer, the author shows that Prozac is a psychiatric tool allowing individuals to better integrate into a society increasingly medicalized.

The article also focuses on the concept of professional self-development, indicating internal and external sources of self-development activity. To deploy the processes of self-development is of great importance level of development of self-esteem. A longitudinal, epidemiological study was carried out in a rural area of Kenya with a population of about 28,000 between 1974 and 1980. Population registration during this time showed that population growth was very high between 1974 and 1978 (4·4% per year) and much lower in 1979 and 1980 (1·1%). Natural increase was nearly as high as in Kenya as a whole (3·7%) in this period. Fertility was somewhat lower than in all Kenya (the crude birth rate was 46 per (...) 1000) while mortality was substantially lower (7 per 1000). Evidence is presented supporting the argument that these low mortality rates are genuine. Levels of temporary and permanent migration are high and probably characteristic for many parts of Kenya. The change in population growth in 1979 and 1980 is probably due to changes in economic conditions leading in particular to less in-migration and to more out-migration. (shrink)

Summary If action theory is to be relevant for the study of social phenomena, its scope has to be enlarged so as to include social structures. A hermeneutic theory of action, which draws on the thoughts of Gadamer, Merleau-Ponty, Ricoeur and Giddens, can meet this requirement. The hermeneutic concept of action, which emphasises the importance of tradition, style and rituals, demonstrates that action and structure presuppose and explain each other. The mutual relationship between action and structure is particularly clear in (...) theories of human development in the domain of psychology, psychotherapy and education. These theories can be used to construct a theory of social evolution, as has been developed by Habermas. By integrating structural, theoretical and critical points of view, a hermeneutic theory of action opens a perspective in which action theory and social theory might be successfully combined. The chapter deals with the contrast between defining aspects of religious rigidity, a socio-historically derived counter-narrative, and anti-consumerism in Rastafarian philosophy and culture on one hand and the universal message and commercial success of the music on the other. After discussing the status of the genre as part of Jamaican national culture, the inherent socio-political claim of Reggae and Rastafarian culture are put in context with the conflicting claims of superiority and non-partiality that can frequently be found in the music. Along the lines of religious doctrines in Rastafarian culture and the attempted spiritual transcendence of racism the chapter closes with an exploration of the genre's signature tropes of social justice and anti-materialism, also considering the less strictly defined identity-constructions of recent artists linked to the Reggae Revival movement. "Martin A. M. Gansinger's study of Reggae and class employs Rastafarianism as a key ingredient of both concepts. Gansinger contends that Rastafarianism offers a "spirituality-based consciousness" that lends itself to an egalitarian-driven social consciousness. Reggae's strong social, political and culture capital as a philosophy of liberation owes much to Rastafarianism, which, for all its contradictions, embodied in categories such as the "righteous" and the "wicked", as well as its segregationalist and patriarchal past, offers a means through which the music advances a socially transformative spirituality that may play a part in the rediscovery of a truly social consciousness." The objectives of this study were to develop a high-density chromosome bin map of homoeologous group 7 in hexaploid wheat, to identify gene distribution in these chromosomes, and to perform comparative studies of wheat with rice and barley. We mapped 2148 loci from 919 EST clones onto group 7 chromosomes of wheat. In the majority of cases the numbers of loci were significantly lower in the centromeric regions and tended to increase in the distal regions. The level of duplicated loci (...) in this group was 24% with most of these loci being localized toward the distal regions. One hundred nineteen EST probes that hybridized to three fragments and mapped to the three group 7 chromosomes were designated landmark probes and were used to construct a consensus homoeologous group 7 map. An additional 49 probes that mapped to 7AS, 7DS, and the ancestral translocated segment involving 7BS also were designated landmarks. Landmark probe orders and comparative maps of wheat, rice, and barley were produced on the basis of corresponding rice BAC/PAC and genetic markers that mapped on chromosomes 6 and 8 of rice. Identification of landmark ESTs and development of consensus maps may provide a framework of conserved coding regions predating the evolution of wheat genomes. (shrink)

To address the complex challenge of global antimicrobial resistance (AMR), a pandemic treaty should include mechanisms that 1) equitably address the access gap for antimicrobials, diagnostic technologies, and alternative therapies; 2) equitably conserve antimicrobials to sustain effectiveness and access across time and space; 3) equitably finance the investment, discovery, development, and distribution of new technologies; and 4) equitably finance and establish greater upstream and midstream infection prevention measures globally. Biodiversity, climate, and nuclear governance offer lessons for addressing these challenges.

It seems that Heidegger's thought is subject to the dubious destiny of inspiring fat commentaries. However, Colombo's has this advantage over many of its counterparts, viz., it is more a true study than a reiteration. This is so in three important respects. First, a systematization of the thought of Heidegger is attempted, and, in so far as this is possible, rather successfully achieved. The book is divided into five parts, whose continuity follows only in part the development of Heidegger. Happily, (...) the author, unimpressed by the superficial schema of a Kehre, articulates a treatment which is independent from any simple genetic explanation. The five parts are: Design and Depth of Finitude, Beyond Form, Manifestation of Being, Man and Nothingness, and The Impotence of Thought. The systematization rests on an excellent discernment of themes in the thought of Heidegger. Instead of being content with the proposition that Heidegger is but a single theme thinker, Colombo rescues a variety of themes from the web of Heidegger's thought. This he does not by formulating such themes in classical or purely technical terms, but by showing their import and relevance relative to other historical themes in Western thought. Surprisingly enough, the result is not an absorption of Heidegger by the tradition, but rather a deepening of classical themes, and a freeing of shades missing in the Heideggerian synthesis. However this is done with such care that the focused topics are not taken out of context, or even hermeneutically overcome. On the contrary, they are very properly accented in their own contours. As befits a good study of this type, footnotes are the best part of the work. Colombo discourses with a multitude of authors, clearing lines of interpretation, and, more importantly, bringing forth ideas generated by Heidegger's philosophy. By being free from the poetic spell that the German philosopher casts upon those who study him, Colombo has written a philosophical book showing more clearly than others the scope and the idiosyncrasies of his subject-author. The results are impressive, and they place this young Italian professor, together with Pöggeler and Birault, at the top of the list of Heidegger's interpreters.—A. M. (shrink)

Fifteen years after the first edition of this comprehensive work, German historicism remains largely and conspicuously in the shadows. The great historico-philological and historico-sociological work produced by, and on the fringes of, this school has given way to specialization. Great polygraphs of the caliber of a Meinecke, a Vossler, a Curtius, a Cassirer, a Croce, or an Auerbach seem to have completely disappeared from the scene. But is the necessity for cultural synthesis that these men stressed any less urgent today (...) than before? One of the major tasks envisioned by thinkers of the early twentieth century was the development of a philosophical anthropology which would consolidate all the preceding work of cultural synthesis. Today this task remains in the far horizon after a series of distinguished but discontinuous achievements. Thus the reappearance of the ambitious work of Professor Rossi is a welcome event. There are now indications that sociology is moving away from the neutral functionalism of the consensus years and is beginning to show some interest in the subconscious and in concrete constellations of values representative of communal forms of consciousness in a situation. Post-industrial society can no longer be examined with the rough tools of micro- or even macro-analysis. Because of their refinements in quantification and their awareness of the most specific features of every particular form of group consciousness, the new middle range methods are akin to the synthetic spirit of the historical school. As middle range research progresses, its results can provide the ontic ground for an ontology of the human condition in its adjustment to the world, and in its communicative-transcendent goals. For this reason, Rossi’s work, though excellent in its expository aspects should be read with care to avoid overemphasizing, as he does, the distance between rationalism and irrationalism that was apparent in the main tendencies of the historical school. Surely it is a worthwhile thing to show the tensions between the relativists such as Simmel and Spengler, the intuitionists such as Windelband and Rickert, and finally the neo-idealists such as Troeltsch and Meinecke. The tension that pulled apart these intellectual currents shows by contrast the great synthetic force operative in the systems of Dilthey and Weber, which provided the dominant inspiration for the whole school. But the fundamental perspective in today’s reconsideration of historicism need not be the tempering of any search for ideals through the insistence on sober methodological limits. A more fruitful emphasis can be placed today on action as a mediator between thought and adjustment, on community as a mediator between society and individuals, and most of all on expression and adventure as mediators between survival and moral absolutes. With his neo-Kantian insistence on the historical method, on the discovery of the historical horizon of the human condition, and on the establishment of the institutional connections within historical periods, Professor Rossi: a) misses the point of the anti-mechanistic efforts of modern philosophy, i.e., the constitution of synthetic a priori judgments in their true home grounds, the field of spontaneous human communication and action; b) restricts the ability on the part of the historical method to penetrate and elucidate the areas of the unconscious and of passive syntheses ; c) both presupposes and trivializes the definition of freedom. Freedom in Rossi’s interpretation of historicism, seems to result from the objective displacement of values, rather than from the dramatic insufficiency of every finite person as it transpires in the highest forms of symbolic communication and interaction.—A. It is very useful for consultation purposes for issues in ontology and contemporary philosophy. The development of thought reflected in these writings constitutes a perfect guide to what is living and what is dead in present philosophical practice. What is unique in Ferrater is the amplitude of the circumstance. Only an extreme methodological flexibility allows him to assimilate such a wide world without (...) abstracting from it. Thus we find in these pages journalism, history of ideas, philosophy of history, science, and ontology. When abstraction is intensified--and this seems to be the inevitable trend as the work progresses--the multiplicity of approach and method remains somehow present; for example, in "Man at the Crossroads," the wealth of historical and cultural data is dominated by a rigorous ontology of historical process and crisis. Conversely, in Being and Death, the already powerful and abstract ontological scheme is enriched by a swarm of data from the sciences and from general and personal experience.--A. M. (shrink)
